Gentoo compiles everything from source, on your machine, with your choices baked in via USE flags. Want a system with no Bluetooth, no printing stack, and every binary tuned to your exact CPU? Set the flags, run emerge, and Portage builds precisely that. No other mainstream distro offers this degree of compile-time control over what your software even contains.

The price is time and study: initial installs are measured in hours (or overnight for large packages), and understanding Portage is a genuine discipline. The payoff is a system optimized and minimized to a degree binary distros cannot match, plus an education in how Linux really fits together. Notably, Gentoo now offers official binary packages too, softening the compile burden where you choose. It remains the ultimate expression of "my machine, my rules."
